Documents required for Fresh Passport – Major change in Name
These are the documents that are required for Fresh Passport when there is a Major change in Name. Apart from address proof and date of birth proof, newspaper clippings of Gazette notification regarding the change in name should be also submitted for issuance of fresh passport
